This assignment made me learn basics that has to be done during clinical practice like history taking, case presentation, communication with patient and dealing with the investigations done. As all of us stuck at home due to present situation this online thing helped me in learning new things and diagnose diseases. The guidance and help by interns made our learning easy

BUDIGAM  The case is : A 20 year old female patient came to OPD  , she is a student.  Her chief complaints  are: Yellow discoloration of sclera Burning sensation b/w lowerlimbs Weakness Loss of appetite Weight loss All these complaints are since 6months  HOPI:  The patient had all the above complaints since 6months and she was asymptomatic 6months ago. She started taking herbal medicine when she developed yellowish discoloration for 2days and stopped .  History of Burning sensation b/w lower limbs is seen over a period of time.  Weakness and weight loss around 10 to12 kgs is also seen.     A CASE OF ANALGESIC NEPHROPATHY This Eblog has been created under the guidance of  Dr MADHUMITHA BUDIGAM The case is : A 72 year old male patient who is a farmer came to OPD. His  CHIEF COMPLAINTS are :- On CKD He has Analgesic nephropathy He has k/c/o hypertension (HTN) since 4 years and he underwent 4 sessions of HD(hemodialysis).   He had h/O consuming NSAID 7yrs back intermittently .     HOPI :-Patient was apparently asymptomatic 7 yrs ago. He gives history of consuming NSAIDs since 7 yrs intermittently because of which he developed chronic kidney disease for which he was on conservative management.  He is a k/c/o hypertension since 4 years. He came  on 19.6.2021 for AKI (secondary to GN) on CKD  ( analgesic nephropathy). He underwent 4 sessions of hemodialysis.
